Soluset with burette is a medical device used to deliver a fixed volume of intravenous fluid at a fixed rate through gravity or pressure pump method in pediatric patients, usually together with other medications. A primary IV solution set has a spike adaptor attached at the distal end of the inline burette set. Soluset is used in both primary and secondary IV lines.
Another common name of soluset is volume control set. Other trade names of volume control set are Volutrol (by Travenol) and Buretrol (by Baxter). Soluset is a trade name of the Abbott brand.
Features of a Soluset
- Sterile and pyrogen free
- The flow is approximately 60 drops per ml
- Has clear, soft, cylindrical and large calibrated burette chamber with graduations
- Burette size is 100ml or 150 ml
- The tube length could be 150 cm, 180 cm or 200 cm.
- Bacteria retentive air inlet with 15 µm fluid filter
- Roller type flow control
- E.O gas sterilization.
- Soft and kink resistant PVC tubing
- Automatic shut off valve that prevents trapping of air in the fluid line when measured volume chamber is empty.
Medical Uses of Soluset
- Soluset is used with IV tubing to deliver intravenous fluid to pediatric patients, as it reduce the risk of fluid overload.
- Helps to ensure the complete intravenous (IV) drug administration to the pediatric patient and deliver the required level of medication.
Because of the huge cost of insulin pumps for the administration of insulin in diabetic patients, soluset, though is meant for pediatric fluid administration, may be an option for administering insulin in adults with diabetes in low income countries.
In a proposal by Taoreed Azeez, Soluset Insuin Solution Treatment Algorithm (SISTA) is a proposed method that is cheaper than insulin pump. Volume control set is easily operated by nurses and may fine-tune insulin administration and optimize glycemic control.
It also reduces wastage and wide glycemic fluctuations when using an IV giving set to administer intravenous insulin, a practice in most low income countries that cannot afford insulin pumps.
How to use a Soluset
The medical expert injects the medicine into the soluset and runs it through the tubing in a timed manner till the soluset is empty of the medication.
An intravenous fluid solution is introduced into the soluset to accommodate for the dead space of the IV tubing. It completes the delivery of medication to the child.
The steps in administering IV fluid through soluset are:
- Review the medication order and check for allergies
- Make sure the medication is compatible with the intravenous fluid (primary fluid)
- Wash your hands thoroughly
- Make sure the infusion site has no patency, i.e., it should be open
- Close the clamps of both below and above the volume chamber of the volume control set.
- Open the air vent on top of the volume control set by turning the clamp
- Insert the spike of the volume control set into the intravenous fluid bag and hang the bag on the IV pole.
- Attach the IV tubing to the volume control set, and affix a needleless cannula at the end of the tubing.
- Open the upper clamp below the intravenous fluid to allow the IV fluid to partially fill the volume chamber. Then close the upper clamp.
- Open the lower clamp to prime tubing and affix the cannula with primary IV fluid. Then close the lower clamp.
- Clean the injection port on the top of the partially filled volume control set with an alcohol swab.
- Inject the medication into port.
- Mix medication and the intravenous solution in the volume control chamber by agitating gently.
- Add the fluid required for dilution from the IV bag by opening the clamp above the volume chamber. Make sure you close the clamp after adding the required amount in the chamber.
- You should note the date, time, medication, and dosage on the medication label. Use labels that can be easily read when you hang the bag.
- Program the infusion pump to deliver medication at the desired infusion rate.
- Open the clamp below volume chamber.
- Manually adjust the drip rate if it is not on IV pump.
